Community Use of School Facilities

CUS ONLINE APPLICATION

The Community Use of Schools Online Application is available at cus.ugdsb.on.ca. In order to access the application you must first log in. If you have trouble logging in please contact us at community.use@ugdsb.on.ca.

For the 2012-2013 season all grandfathered applications must be submitted online by June 30, 2012. Any grandfathered application received after June 30th will not be processed.

COMMUNITY USE OF SCHOOLS HANDBOOK

The CUS Handbook has been prepared to answer your questions about the Community Use of Schools Program at the Upper Grand District School Board. It is expected that every applicant will read the CUS Handbook as a condition of use.

Please refer to the handbook for any questions. Should you have a question not answered by the CUS Handbook please contact the CUS Clerk at 519-822-4420 xt 1833. We would appreciate hearing any ideas for further information needed in the CUS Handbook.

COMMUNITY USE ONLINE BOOKING WEBSITE

VIEWING AVAILBLE FACILITIES

You will have the ability to view available facilities online.

Schools are not available to be booked for CUS during regular school hours. Due to the nature of our booking software it may appear that school facilities are available for CUS during regular school hours. This is not the case, CUS is not available until after 6PM in elementary schools and 7PM in secondary schools. If you wish to book a time for a CUS event during regular school hours please contact the CUS Clerk at 519-822-4420 xt.1833.

When using the Facility Availability Search feature of the website you can look up what school facilities are available for booking. By early September we will have the school facility availability inputted and as up to date as possible for the upcoming school year. But due to our booking process there may be a ten day delay between what the website says is available and what is actually available. Please do not assume that your booking is finalized until you have received your CUS Permit or received confirmation from the Board.

THE APPLICATION

The CUS Application is a writable PDF that can be saved to your hard drive as a completed form. The CUS Application is similar to the Board’s paper application that has been in use for the last few years. The CUS Application can be emailed directly using your standard email software (Outlook, etc) or the CUS Application can be saved to your computer and emailed as an attachment. Email the CUS Application to community.use@ugdsb.on.ca.

INSURANCE

A certificate with a minimum of $2,000,000 liability insurance naming the Upper Grand District School Board as an additional insured, must be received at the Board office within ten business days of submitting a CUS Application. Should your CUS event occur within the ten business days of applying, your insurance certificates must be in the Board office the same day as your CUS Application.

If possible, send in the insurance certificate as an attachment to the email with your CUS Application.

LOGIN

In order to fill out a CUS Application you must first login on the community use website. If you are a previous client then you already have an account with the UGDSB, do not create a new account. In order to activate your online account you must forward us an email address to community.use@ugdsb.on.ca so that you can receive your login information. Once we have attached your email address to your account you can request your Login ID and PIN by clicking the Forgot Password button on the Login Page.

Once you have logged in you can check your account status, make online payments and fill out a CUS Application.

If you are new to CUS bookings with the Board, please set up an account using the create an account setting. When filling out the CUS Application you will have to put the same email address and name as the account to ensure your booking.

PAYMENT

Payment is expected in full upon receipt of your Permit. Payment may be made online through the CUS website or by cheque. All CUS Permits have a non-refundable Administration Fee of $28.25 ($25.00+HST). Any changes to a CUS Permit will result in an additional Administration Fee charged to your account.
